THE ABDOMINAL CAVITY AND PERITONEUM

Introduction

  1. Liver: The largest organ in the abdominal cavity, the liver is positioned just below the diaphragm on the right side. It’s responsible for various metabolic functions, including detoxification, nutrient processing, and bile production.
  2. Stomach: The stomach is located beneath the diaphragm and on the left side of the abdomen. It plays a crucial role in the digestion of food and the initial breakdown of nutrients.

  7. Gallbladder: The gallbladder is a small organ located beneath the liver. It stores and concentrates bile produced by the liver before releasing it into the small intestine to aid in digestion.
  8. Kidneys: The kidneys are located in the upper part of the abdominal cavity, towards the back. They filter blood to remove waste products and regulate fluid and electrolyte balance.
  9. Adrenal Glands: These small glands sit on top of the kidneys. They produce hormones that regulate stress response and metabolism.
  10. Bladder: The urinary bladder is situated in the lower part of the abdomen, just above the pelvic region. It stores urine before elimination.
  11. Reproductive Organs: In females, the uterus, ovaries, and fallopian tubes are located in the lower abdomen and pelvis. In males, the testes are positioned outside the abdominal cavity, while the prostate gland surrounds the urethra near the bladder.

Peritoneal Folds & Ligaments

Folds of the peritoneum are formed by double layers of the membrane that come together to enclose certain structures or create pockets. These folds serve to support and stabilize organs, provide pathways for blood vessels and nerves, and prevent friction between organs. Some notable peritoneal folds include:

  1. Greater Omentum: This is a large, apron-like fold of peritoneum that hangs down from the stomach and transverse colon. It’s often referred to as the “policeman of the abdomen” because it can migrate to areas of infection or inflammation to help contain them.
  2. Lesser Omentum: This fold connects the lesser curvature of the stomach and the proximal part of the duodenum to the liver. It contains the portal triad, which consists of the common bile duct, hepatic artery, and portal vein.
  3. Mesentery: The mesentery is a fold of peritoneum that attaches the small intestine to the posterior abdominal wall. It contains blood vessels, lymphatics, and nerves that supply the intestines.
  4. Mesocolon: This fold supports the large intestine (colon) by attaching it to the posterior abdominal wall. There are different sections of the mesocolon, such as the transverse mesocolon and sigmoid mesocolon.

Ligaments of the peritoneum are double layers of the membrane that connect organs to each other or to the abdominal wall. They play a role in maintaining the position of organs and facilitating their movement. Some prominent peritoneal ligaments include:

  1. Hepatogastric Ligament: Connects the liver to the lesser curvature of the stomach.

Intra-abdominal and retroperitoneal relations

  3. Pancreas: While most of the pancreas is retroperitoneal, its head is located within the peritoneal cavity.
  4. Duodenum: The first part of the small intestine. Its initial segment is intraperitoneal, but the majority is retroperitoneal.
  5. Ascending and Descending Colon: Both are retroperitoneal parts of the large intestine, as is the rectum.
  6. Ureters: These tubes that carry urine from the kidneys to the bladder travel retroperitoneally.
  7. Major Pelvic Blood Vessels: Structures like the iliac arteries and veins are retroperitoneal.

The lesser omentum and greater omentum

    • Rectovesical Pouch (of Douglas in males) / Rectouterine Pouch (of Douglas in females): This is the lowest point in the peritoneal cavity when a person is in a standing position. It’s located between the rectum or uterus and the bladder.
