GLUCOCORTICOIDS PHARMACOLOGY SUMMARIZED

  • Metabolic effects: Glucocorticoids stimulate the breakdown of proteins and fats, resulting in the release of amino acids and free fatty acids into the blood. They also increase blood glucose levels by stimulating gluconeogenesis and reducing glucose uptake by peripheral tissues.
  • Mineralocorticoid effects: High doses of glucocorticoids can cause salt and water retention, leading to hypertension and edema. This is because glucocorticoids also have some mineralocorticoid activity, which is normally balanced by the action of aldosterone.

Glucocorticoids are available in a variety of formulations, including oral tablets, inhalers, topical creams and ointments, and injectable solutions. The choice of formulation and dose depends on the specific condition being treated and the desired pharmacological effect. However, long-term use of glucocorticoids can lead to a number of adverse effects, such as osteoporosis, hypertension, diabetes, and increased susceptibility to infections. Synthetic glucocorticoids are designed to have increased potency and duration of action, as well as decreased mineralocorticoid activity, which can lead to sodium and water retention. Common synthetic analogs include prednisone, prednisolone, dexamethasone, and hydrocortisone.

 

Routes of administration

Rationale of replacement therapy

Glucocorticoid replacement therapy is used to treat adrenal insufficiency, a condition in which the adrenal cortex fails to produce sufficient amounts of glucocorticoids. Replacement therapy aims to restore normal physiological levels of glucocorticoids and prevent the development of symptoms such as fatigue, weakness, and hypotension.
