Causes of Otitis Media

The reason for classification of otitis media

Acute otitis media (AOM) is a bacterial or viral infection of the middle ear that typically causes sudden onset of pain, fever, and other symptoms such as ear discharge, hearing loss, and difficulty sleeping. The treatment of AOM depends on the severity of the symptoms and the age of the patient. In children over six months of age, mild to moderate AOM may be managed with observation and pain relief. Antibiotics may be prescribed if the symptoms are severe or if the patient is under two years of age, as they are at higher risk for complications such as hearing loss and delayed language development.

OME is common in children and often resolves on its own within a few months. Otitis media Investigations

Audiological and radiological investigations are often used to diagnose and evaluate these complications.

a) Audiological investigations:

  1. Pure-tone audiometry: This test measures the patient’s hearing ability in response to different sound frequencies and intensities. It can help diagnose hearing loss and determine its severity.
  2. Tympanometry: This test measures the movement of the eardrum in response to changes in air pressure. It can help diagnose middle ear fluid and eardrum perforations.
  3. Acoustic reflex testing: This test measures the contraction of the middle ear muscles in response to loud sounds. It can help diagnose nerve damage and middle ear disorders.

 

b) Radiological investigations:

  1. X-rays: X-rays are not routinely used in the diagnosis of otitis media and its complications, but they can be used to evaluate the sinuses and mastoid bone.
  2. Computed tomography (CT) scan: CT scan can provide detailed images of the temporal bone, including the middle ear and mastoid. It can help diagnose mastoiditis and bone erosion.
  3. Magnetic resonance imaging (MRI): MRI can provide detailed images of soft tissues, including the brain and inner ear. It can help diagnose complications such as meningitis and facial nerve palsy.

  1. Hearing Loss: Cholesteatoma can cause hearing loss by damaging the delicate structures of the ear, including the ossicles and the cochlea. Treatment may involve hearing aids, reconstructive surgery, or cochlear implants, depending on the severity of the hearing loss.
  2. Balance Problems: Cholesteatoma can cause vertigo, dizziness, and balance problems, particularly if the growth has damaged the inner ear or the nerve that controls balance. Treatment may involve medication, physical therapy, or surgical repair of the inner ear.
