Detailed Medical History: Assess menstrual history, fertility issues, and any signs of virilization (deepening voice, male pattern baldness, clitoromegaly). ii. Blood Tests: Measure hormone levels, including testosterone, free androgen index (FAI), dehydroepiandrosterone sulfate (DHEAS), sex hormone-binding globulin (SHBG), and 17-hydroxyprogesterone (17-OHP) to assess ovarian and adrenal function. B. Systemic Virilization: When hirsutism is accompanied by other signs of virilization, it suggests a more systemic and potentially severe underlying cause. The initial workup for systemic virilization includes:
  • Detailed Medical History: As in cutaneous virilization, gather information on menstrual history, fertility, and signs of virilization.
  • Physical Examination: In addition to assessing hair distribution, examine for signs of virilization such as clitoromegaly, deepening of the voice, temporal balding, and muscular hypertrophy.
