The term “sero-negative” refers to the absence of specific antibodies in the blood, such as rheumatoid factor (RF) and anti-citrullinated protein antibodies (ACPAs), which are typically associated with other types of arthritis like rheumatoid arthritis. Sero-negative spondyloarthropathies include conditions such as ankylosing spondylitis, psoriatic arthritis, reactive arthritis, enteropathic arthritis, and undifferentiated spondyloarthritis.

A) Clinical Features: The clinical features of sero-negative spondyloarthropathies can vary widely among individuals and may include:

  1. Inflammatory back pain: This is a hallmark symptom of ankylosing spondylitis and is characterized by insidious onset, improvement with exercise, and waking up in the second half of the night due to pain.
  2. Peripheral arthritis: Inflammation and swelling of joints, commonly involving the lower extremities in asymmetric fashion.
