POSTPARTUM HEMORRHAGE AND ITS IMPACT ON MATERNAL HEALTH

Postpartum hemorrhage (PPH) refers to excessive bleeding following childbirth. It is a significant cause of maternal morbidity and mortality worldwide. There are two main classifications of PPH: primary and secondary.

  1. Primary Postpartum Hemorrhage (PPH): Primary PPH refers to excessive bleeding that occurs within the first 24 hours after childbirth. It is usually caused by issues related to the birth process or the immediate postpartum period. The most common causes of primary PPH include:

a) Uterine atony: This is the failure of the uterine muscles to contract properly after delivery, leading to inadequate closure of the blood vessels. It is the most common cause of primary PPH.

b) Trauma to the birth canal: Tears or lacerations in the vagina, cervix, or perineum can result in significant bleeding.

c) Retained placenta or placental fragments: If the placenta is not completely expelled after delivery, it can cause bleeding.

d) Uterine rupture: A rare but severe complication where the uterus tears during labor or delivery, leading to massive bleeding.

e) Coagulation disorders: Certain medical conditions or medications can affect blood clotting, increasing the risk of bleeding.

Prompt identification and management of primary PPH are crucial to prevent severe complications and ensure the well-being of the mother.

  1. Secondary Postpartum Hemorrhage: Secondary PPH refers to excessive bleeding that occurs after the first 24 hours and up to six weeks postpartum. It is less common than primary PPH but can still be serious. The causes of secondary PPH include:

a) Retained placental tissue: If fragments of the placenta remain in the uterus, they can cause bleeding in the days or weeks following childbirth.

b) Infection or endometritis: Inflammation of the uterine lining can lead to bleeding.

c) Subinvolution of the uterus: The uterus does not return to its normal size and function as expected, resulting in persistent bleeding.

d) Uterine artery pseudoaneurysm: A rare condition where a weakened area of the uterine artery forms an abnormal bulge and may rupture, causing bleeding.

e) Coagulation disorders: Similar to primary PPH, coagulation disorders can contribute to bleeding in the postpartum period.

Management of secondary PPH involves identifying and addressing the underlying cause to stop the bleeding and prevent complications.

Causes of Postpartum hemorrhage

Postpartum hemorrhage (PPH) refers to excessive bleeding following childbirth, typically defined as blood loss of 500 milliliters or more within 24 hours after vaginal delivery, or 1,000 milliliters or more after a cesarean section. PPH can have multiple causes, including:

  1. Uterine atony: This is the most common cause of PPH. It occurs when the uterus fails to contract adequately after delivery, leading to poor compression of blood vessels at the placental site. Uterine atony can result from a variety of factors, such as overdistention of the uterus (as in cases of multiple pregnancies or large baby), prolonged labor, use of certain medications (e.g., magnesium sulfate), or maternal medical conditions (e.g., preeclampsia).
  2. Genital tract trauma: Injury to the birth canal, including the vagina, cervix, or perineum, can cause significant bleeding. Tears or lacerations can occur during childbirth, particularly if an episiotomy (surgical cut to enlarge the vaginal opening) is performed or if instrumental delivery methods (e.g., forceps or vacuum extraction) are used.
  3. Retained placental tissue: Failure of the placenta or its fragments to completely separate and be expelled from the uterus can result in continued bleeding. This can occur due to an adherent placenta (placenta accreta, increta, or percreta), retained placental fragments, or an inverted uterus.
  4. Coagulation disorders: Certain medical conditions or medications can impair the blood’s ability to clot properly, increasing the risk of bleeding. Examples include hemophilia, von Willebrand disease, thrombocytopenia, liver disease, or the use of anticoagulant medications.
  5. Uterine rupture: Although rare, a tear or rupture in the uterine wall during labor or delivery can cause severe bleeding. Uterine rupture is more common in women who have had a previous cesarean section or uterine surgery.
  6. Placenta previa: In cases where the placenta is implanted low in the uterus and partially or completely covers the cervix, bleeding can occur before, during, or after delivery. Placenta previa increases the risk of PPH.
  7. Uterine inversion: This occurs when the uterus turns inside out, typically during the third stage of labor when the placenta is delivered. Uterine inversion can lead to profuse bleeding and is a rare but severe cause of PPH.
  8. Maternal factors: Certain maternal conditions can increase the risk of PPH, such as high blood pressure (preeclampsia), obesity, advanced maternal age, multiple pregnancies (e.g., twins or more), or a history of previous PPH.

Management of massive Postpartum hemorrhage

Managing massive postpartum hemorrhage requires a prompt and coordinated approach involving various medical professionals. Here’s an overview of the management strategies for massive PPH:

  1. Early recognition and prevention: Early identification of women at risk for PPH is crucial. Antenatal risk assessment should be conducted, and any predisposing factors should be addressed, such as anemia, placental abnormalities, or a history of PPH. Additionally, active management of the third stage of labor (administering uterotonic drugs, controlled cord traction, and uterine massage) can help prevent PPH.
  2. Initial assessment and resuscitation: The immediate priority is to stabilize the patient’s vital signs. This includes ensuring a patent airway, administering supplemental oxygen, establishing intravenous access, and obtaining blood samples for laboratory investigations. Crystalloid fluids or blood products may be given to correct hypovolemia.
  3. Uterine massage and bimanual compression: Gentle uterine massage can help stimulate uterine contractions and control bleeding. If bleeding persists, bimanual compression may be performed, where a hand is inserted into the vagina to apply pressure on the uterus while the other hand compresses the uterus from above the abdomen.
  4. Uterotonic agents: Uterotonic drugs such as oxytocin, ergometrine, and prostaglandins (e.g., misoprostol) are administered to enhance uterine contractility and control bleeding. These medications can be given intravenously, intramuscularly, or directly into the uterine cavity.
  5. Surgical interventions: If conservative measures fail, more invasive interventions may be required. These can include: a. Uterine balloon tamponade: A balloon catheter is inserted into the uterine cavity and inflated to exert pressure and control bleeding. b. Uterine artery embolization: Interventional radiologists can selectively occlude the uterine arteries, reducing blood supply to the uterus and controlling bleeding. c. Other surgical interventions: In severe cases, surgical options like uterine artery ligation, uterine compression sutures (e.g., B-Lynch suture), or even hysterectomy may be necessary to achieve hemostasis.
  6. Supportive care: In addition to the medical interventions, supportive care is crucial. This includes close monitoring of vital signs, frequent blood tests, blood transfusion as required, and correction of coagulation abnormalities.
  7. Complications of Postpartum hemorrhage

  1. Hypovolemic shock: Severe bleeding can lead to a rapid loss of blood volume, causing a drop in blood pressure and inadequate perfusion of vital organs. This can result in hypovolemic shock, which is a medical emergency.
  2. Anemia: Significant blood loss can lead to anemia, a condition characterized by a decrease in the number of red blood cells or hemoglobin levels. Anemia can cause fatigue, weakness, shortness of breath, and other symptoms.
  3. Organ dysfunction: Insufficient blood supply to vital organs can lead to organ dysfunction or failure. The brain, heart, kidneys, and liver are particularly vulnerable to damage due to inadequate perfusion.
  4. Disseminated intravascular coagulation (DIC): In some cases of severe PPH, the body’s clotting system may become overactive, leading to disseminated intravascular coagulation. This condition involves widespread blood clotting in small blood vessels throughout the body, which can result in organ damage and further bleeding.
  5. Infection: Prolonged or severe postpartum bleeding can increase the risk of infection. Open wounds, such as a lacerated uterus or perineal tear, may serve as entry points for bacteria, leading to infections like endometritis or wound infections.
  6. Maternal death: Although rare in developed countries, severe postpartum hemorrhage can result in maternal death if not promptly treated. Immediate medical intervention is crucial to prevent this outcome.

    • Balloon Tamponade: In cases where uterine atony persists despite medical management, a balloon tamponade device (e.g., Bakri balloon or Foley catheter) may be placed in the uterine cavity to compress bleeding vessels and promote hemostasis.
    • Uterine Artery Embolization (UAE): If other measures fail, UAE can be considered. It involves the selective occlusion of uterine arteries to control bleeding.

    • Manual Removal: Manual exploration of the uterus is performed to identify and remove any retained placental fragments.
    • Curettage: If manual removal is unsuccessful or incomplete, a surgical procedure called curettage may be performed. It involves scraping the uterine lining to remove the remaining placental tissue.
    • Hysterectomy: In rare cases of severe and life-threatening PPH due to retained placental tissue, a hysterectomy (surgical removal of the uterus) may be necessary.
  4. Coagulation Disorders: In some cases, PPH may occur due to abnormal clotting mechanisms or coagulation disorders. The management options include:
    • Blood Products: Administration of blood products, such as packed red blood cells, fresh frozen plasma, platelets, or clotting factors, is done to correct coagulopathies and replace blood loss.
    • Pharmacological Therapy: Medications like tranexamic acid may be used to promote clot formation and prevent further bleeding.
    • Specific Coagulation Factor Replacement: In cases of known coagulation disorders, specific replacement of deficient clotting factors may be required.
