EPIDEMIOLOGY AND PATHOPHYSIOLOGY OF AUTOIMMUNE HEPATITIS

Autoimmune hepatitis (AIH) is a chronic liver disease characterized by immune-mediated inflammation and damage to the liver cells (hepatocytes). It is considered an autoimmune disorder because the immune system mistakenly attacks the liver cells as if they were foreign substances.

Autoimmune hepatitis is a progressive condition that primarily affects young to middle-aged individuals, but it can occur at any age. It is more common in women than in men. AIH is characterized by the presence of autoantibodies in the blood, elevated liver enzyme levels, and histological evidence of interface hepatitis (inflammation at the interface between hepatocytes and portal tracts) on liver biopsy. If left untreated, autoimmune hepatitis can lead to cirrhosis and liver failure.

Epidemiology: The exact cause of autoimmune hepatitis is unknown, but it is believed to result from a combination of genetic and environmental factors. The condition is relatively rare, with an estimated incidence of 1 to 2 cases per 100,000 individuals per year. It can affect people of all ethnicities, although certain populations, such as individuals of Northern European or Middle Eastern descent, appear to have a higher risk. Additionally, there may be a genetic predisposition, as autoimmune hepatitis is more common in individuals with a family history of autoimmune diseases.

Pathophysiology: The exact pathophysiology of autoimmune hepatitis is not fully understood, but it is thought to involve an aberrant immune response directed against the liver cells. It is believed that a combination of genetic susceptibility and environmental triggers leads to the activation of autoreactive T cells, which play a central role in the immune response.

In autoimmune hepatitis, T cells mistakenly recognize liver antigens as foreign and initiate an immune response against them. This immune response leads to the production of autoantibodies, such as anti-nuclear antibodies (ANA), anti-smooth muscle antibodies (ASMA), and anti-liver kidney microsomal antibodies (LKM-1). These autoantibodies contribute to the inflammation and destruction of liver cells.

The liver inflammation in autoimmune hepatitis is characterized by the infiltration of immune cells, including T cells, B cells, and macrophages, into the liver tissue. These immune cells release various inflammatory mediators, such as cytokines and chemokines, which further perpetuate the immune response and cause liver damage.

ASMA titers can vary and may not correlate with disease severity. c. Anti-liver kidney microsome type 1 antibodies (anti-LKM-1): Anti-LKM-1 antibodies target a specific liver and kidney antigen called cytochrome P450 2D6. These antibodies are typically found in type 2 AIH, which is more common in children and adolescents. The presence of anti-LKM-1 antibodies is highly specific for type 2 AIH. d. Anti-liver cytosol type 1 antibodies (anti-LC1): Anti-LC1 antibodies are directed against a soluble liver antigen. They are often found in association with anti-LKM-1 antibodies and are more specific to type 2 AIH.
  2. Non-specific markers: a. Elevated levels of serum immunoglobulin G (IgG): Patients with AIH often have increased levels of IgG, particularly IgG1 and IgG3 subclasses. Elevated IgG levels can support the diagnosis of AIH. b. Hypergammaglobulinemia: AIH is commonly associated with increased levels of multiple immunoglobulins, leading to hypergammaglobulinemia. c. Liver enzymes: Serum liver enzymes, such as alanine aminotransferase (ALT) and aspartate aminotransferase (AST), are often elevated in AIH, indicating liver inflammation and damage.

It’s important to note that the presence of these serological markers alone is not sufficient for diagnosing AIH. Clinical evaluation, liver biopsy, and exclusion of other liver diseases are also necessary for a definitive diagnosis. Additionally, the serological markers may vary among individuals, and their absence does not exclude the diagnosis of AIH.

 

Manifestations of autoimmune hepatitis

  5. Elevated liver enzymes: Autoimmune hepatitis leads to increased levels of liver enzymes such as alanine aminotransferase (ALT) and aspartate aminotransferase (AST). Elevated liver enzyme levels can be detected through blood tests.
  6. Joint pain: Some individuals with autoimmune hepatitis may experience joint pain and stiffness, similar to symptoms seen in other autoimmune conditions like rheumatoid arthritis.

  8. Spider angiomas: Spider angiomas are tiny, red blood vessels that resemble a spider’s web. They can appear on the skin, particularly on the face, chest, and arms. Spider angiomas can be a manifestation of autoimmune hepatitis.
  9. Digestive issues: Digestive symptoms such as nausea, vomiting, loss of appetite, and weight loss can occur in autoimmune hepatitis. These symptoms are often related to the liver’s role in digestion and nutrient processing.
  10. Generalized malaise: People with autoimmune hepatitis may experience a general feeling of illness or malaise. This can include symptoms like fever, night sweats, and overall discomfort.

  3. Budesonide: In some cases, budesonide may be used instead of prednisone as a corticosteroid option. It has a more localized effect in the liver and may have fewer side effects than prednisone, but its effectiveness may vary depending on the individual.
  4. Immunosuppressive drugs: If corticosteroids and azathioprine are not effective or tolerated, other immunosuppressive medications may be considered, such as mycophenolate mofetil, cyclosporine, or tacrolimus. These medications suppress the immune system and reduce liver inflammation.
  5. Ursodeoxycholic acid (UDCA): UDCA may be prescribed as an adjunctive therapy in some cases. It helps to improve liver function and has a potential immunomodulatory effect.
  6. Liver transplantation: In severe cases of autoimmune hepatitis that do not respond to medication or have progressed to end-stage liver disease, liver transplantation may be necessary. Transplantation offers the best chance of long-term survival and a cure for autoimmune hepatitis.

Liver Transplant Indications

It is typically considered when other treatment options have been exhausted and the patient’s liver is no longer able to perform its essential functions. The decision to undergo a liver transplant is based on several indications, which include:

  1. End-stage liver disease: Liver transplantation is considered for individuals who have reached the end stage of liver disease, where their liver function is severely compromised and irreversible. This may result from chronic conditions such as cirrhosis, chronic hepatitis B or C, alcoholic liver disease, primary biliary cirrhosis, primary sclerosing cholangitis, or genetic disorders like Wilson’s disease or hemochromatosis.
  2. Acute liver failure: In cases of sudden and severe liver failure, where the liver rapidly loses its function within a short period, a liver transplant may be necessary. Acute liver failure can occur due to drug-induced liver injury, viral hepatitis, autoimmune hepatitis, or other causes.
  3. Hepatocellular carcinoma (HCC): Liver transplantation can be an option for selected patients with hepatocellular carcinoma, which is the most common type of liver cancer. It is considered when the tumor is within specific size and number criteria and meets other established criteria for transplantation.
  4. Cholangiocarcinoma: This is a type of cancer that originates in the bile ducts within or outside the liver. Liver transplantation may be considered for certain cases of cholangiocarcinoma that meet specific criteria, including tumor size, location, and absence of spread beyond the liver.
  5. Inborn errors of metabolism: Some genetic disorders affect the liver’s ability to metabolize substances properly. In certain cases, liver transplantation can be a treatment option for individuals with conditions such as alpha-1 antitrypsin deficiency, Crigler-Najjar syndrome, glycogen storage diseases, or urea cycle disorders.
  6. Recurrent liver disease: In some cases, individuals may experience a recurrence of liver disease after undergoing a prior liver transplant. If the recurrence progresses and threatens the function of the transplanted liver, a re-transplantation may be considered.
